Sunday School Registration
Submit one form per student, age 2 through Senior year of high school. Student must be 2 years old by September 1.
Student, mother, and father addresses are all required. Use “n/a” if there is only one parent. Parent’s addresses may also be listed as “same”.
We send a lot of information via email. Please give us an email address that will reach the student or parent.
The information on the form will remain after it has been submitted. You can change the child information and register another child.
